As both forms are required, Please print out both the HTML and the PDF Deafness form:

HTML: Instructions and Form to be completed by Owner and Veternarian,and submitted to GDC. (2 Pages)

PDF: The form for the Veterinarian to enter all the test results on the litter. The Form is one page in landscape format and Acrobat Reader3.0 or 4.0 is required. If Acrobat is not available, please contact GDC for a faxed or mailed copy of the form

NB: Not all browsers will print out the same. If the printout is not satisfactory, please contact GDC.

Some of the forms and technical articles will be in the Adobe Acrobat PDF format. If you do not have the Acrobat Reader software please go to the Adobe site and download their free software. It is well worth the effort as many sites now use the PDF format.

The file is large, 4.9MB and may take as long as 25 minutes. Though the PDF version of the form will more closely resemble the GDC hard copy, you may prefer to print and use the HTML version of the forms, if they exist.
